By submitting this form, you will become a BCFSG member.

BCFSG will retain personal or health information provided by you. BCFSG may provide that information to Bendigo Health Carer Support Services, if requested, for the purpose of providing further support and respite services to BCFSG members.

Subsidies

Bendigo CF Support Group offers a range of support services for our members. Select one of the below subsidies, download and fill out the form, and email it to [email protected].

CF-Related Hospital Admission & Clinic Visit: We offer this subsidy to our members to offset the cost of travelling and staying in Melbourne for CF-related clinic visits and hospitalisations.

CF-Related Expenses: This subsidy aims to support our member families and adult CF members with their ongoing expenses related to CF.

Respite: This subsidy is designed to assist our member families and adult CF members to ‘take a break’ and enjoy some respite.

Adult CF Pharmaceutical: This subsidy helps reduce the effective Medicare Safety Net by 25% for CF adults who do not have a Health Care Card.
